as referenced by Tina Urbaitis, Appeal Coordinator NYS Education Department, October 26, 2016, March 16, 2016, October and November 2015 that I want to submit with my section 276.5 application are:


Report as related to: Liars/Public Trust Abusers Vincent J. Coppola and Daniel J. D'Amico: As reported on January 14, 2015 receiving a report from hearing officer D'Amico, Vincent J. Coppola never received a report stating:



Notice of Suspension Report as related to: Liars/Public Trust Abusers John Crangle, Andrew J. Freedman and Daniel J. D'Amico: HCSD has never attempted to assure deliver a NOTICE OF SUSPENSION to anyone stating as John Crangle read during the February 10, 2015 superintendent's hearing: "Notification by phone was attempted because I did contact home regarding these suspension consequences and while the student is suspended they are not permitted on school property nor can they attend any school sponsored events and if they had any questions they were to contact the Superintendent's Office” and “suspension which was to take place on December 22nd and December 23rd"

Surveillance video evidence as related to: Liar/Public Trust Abuser Mike Cornell: "Mr. Mike Cornell, Superintendent of Schools called me on June 13, 2016 and we did have an opportunity to speak on June 15, 2016. He stated that they could not share the (surveillance) video with Daniel at this time".

Attendance and grade academic records as related to: Liar/Public Trust Abusers superintendent Mike Cornell, interim superintendent Vincent J. Coppola and Andrew J. Freedman: The attendance and grade records of teachers Zdrojewski, Tangelder, Booth, Tretter, Lauria, Bogner, Murray, Warner and Day promised by Andrew J. Freedman:

and Director NYS Department of Education Deborah Marriott October 19, 2015. These attendance and grade records were investigated by NYS Acting Commissioner for Office of Children and Family Services Sheila J. Poole. On March 17, 2015 acting Commissioner Poole ruled interim superintendent Coppola’s and Andrew J. Freedman’s February 5, 2015 claim had no credible evidence; thus one big lie. The February 5, 2015 one big LIE of interim superintendent Coppola and Andrew J. Freedman was "Daniel (age 13) has missed 39 days of school and is failing. The parents Dave and Olena are aware of this and they are not addressing the child’s educational needs".


The informal conference result that NYS regulations and HCSD Code of Conduct mandated to transact December 18, 2014 and has yet to transact. A section 276.5 application will be immediately made when the following and prior requested HCSD obstructed fact of Tina Urbaitis is made available to include in this application:

The above D'Amico report is received.
The above NOTICE OF SUSPENSION is received.
The surveillance video is made available for viewing.
The attendance and grade records investigated by Acting Commissioner Poole are received.
The informal conference is transacted.

It is believed there is no D'Amico report, no NOTICE OF SUSPENSION as described above and no surveillance video verifying Daniel's reported act of misconduct.


It is believed the attendance and grade records will validate interim superintendent Coppola’s and Andrew J. Freedman’s February 5, 2015 intentional dishonesty.

It is believed that the informal conference questioning will validate Ms. McGirr's dishonesty.

Commissioner Elia please take steps to make the above 5 points available or rule the 5 points mischievous.
